Payment, Shipping & Refund Policy

The following policies apply for patients that are seen via Video Appointment: “Video Appointment” means that you may be evaluated and treated by a health care provider from a different location via electronic communication.

PAYMENT POLICY:

ALL PAYMENT IS EXPECTED AT THE TIME OF SERVICE, however, some services may require a deposit in advance. We only accept payment in the form of VISA, MasterCard, American Express or Discover, and Advance Care Card. Patient’s card on file will be charged for any services received and prescription medications being ordered at the conclusion of the video appointment.

Fees for prescription medications prescribed under our Weight Loss Programs will be collected at the time of your appointment, and transmitted directly to Nationwide Compounding Pharmacy unless you choose to have a written prescription for medications provided to you for fulfillment at the pharmacy of your choice. If you choose to receive a written prescription for medications, please be advised that the full program fee will still apply.

SHIPPING POLICY:

All medications prescribed during a video appointment will be shipped to your home address on file. Nationwide Pharmacy will prepare and ship all medications through a third party carrier. Patients within Arizona may also specifically request their medications be delivered to one of our office locations for pick up. Standard shipping will apply and can take up to one week. Expedited shipping is available for an additional charge. Please let your Provider know your preference at the time of checkout.

REFUND POLICY:

Prescription medications and certain medical grade supplements are subjected to state and federal laws. These laws do not permit us to restock items or to accept returned prescription medications for refund. Therefore, ALL SALES ARE FINAL.
